Conflict of Interest Disclosure Form
Use this form to identify a situation where you are concerned whether you or a family member may have a conflict between your personal interests and your responsibilities at DU. The Office of Enterprise Risk Management will work with you to determine whether an actual conflict exists and, if so, what steps should be taken to resolve the conflict.
Conflict of interest situations can often be resolved by recusing the conflicted individual from decision-making related to the subject of the conflict. Information provided is maintained in a confidential manner, subject to the procedures outlined in the Conflict of Interest Policy.
