Household Confirmation Form

This form is required for students who have been selected for an institutional review of their financial aid application, and is used to confirm the number of people in the student's household and the number of family members in college. Please download and submit the form to our office.

Note: The parent information you report should be for the parent(s) that completed the FAFSA.

• 2022-23 Household Confirmation Form (for students applying for the fall of 2022 or later)
• 2021-22 Household Confirmation Form (for students applying for the winter, spring, or summer of 2022)
